The impact of pregnancy termination on a woman’s psychological state.

The wide distribution of abortions, the attitude towards abortion in culture, the problems of psychological adaptation of women who decide to have an abortion, as well as the consequences of a violation of the psychological state of women after termination of pregnancy - all this determines the need for a deep psychological consideration of the motives and experiences accompanying the abortion.

In Ukraine in 2000, there were 112.9 abortions per 100 births (according to the State Statistics Committee of Ukraine).

Various aspects of the abortion problem are analyzed in the medical and sociological literature. As for psychological research, they seem to be pushing this problem to the periphery of their attention. And if in Western literary sources one can find some studies concerning the problem of studying disorders in the psychological state of women after termination of pregnancy, then in the domestic scientific and psychological literature there are none.

One of the newest American diagnostic textbooks, The Diagnostic and Statistical Manual, DSM III, describes a group of disorders associated with stress after suffering psychological trauma. Based on the violations listed there, it is customary in the scientific literature to say that one of the mentioned stresses that exceeds “ordinary human experience” is abortion - the artificial termination of pregnancy.

The experience of abortion, that is, the deliberate destruction of one’s own conceived child, causes trauma and causes serious changes in the woman’s psyche. As a result, a woman may consciously or unconsciously return to this event in her life.

From the point of view of psychiatry, women who decide to terminate a pregnancy under the influence of the current situation, that is, all those women who terminate a pregnancy for so-called social reasons, are in a depressive-anxious state. Their mood is dominated by fear, their will is paralyzed, they feel pressure on themselves, they feel helpless, that is, they show symptoms of a typical depressive syndrome - they see no other way out. The range of various psychological complications caused by abortion trauma is very wide: from minor psychological disorders to serious mental disorders.

Initially, after a pregnancy loss, women develop psychological defense mechanisms that temporarily alleviate their trauma. However, when the defense system fails, women begin to suffer. Insomnia, terrible dreams, lack of appetite, disruption of biological rhythm, causeless crying, sudden mood changes - all this is typical for women after an abortion. Often they begin to experience psychosomatic disorders simultaneously with migraines, pain in the lower abdomen, etc. There may also be mental disorders that manifest themselves in withdrawal from people, excessive use of medications and alcohol. Some women become distrustful, others become paranoid to the point of developing psychosis.

In Grenoble (1971), at a symposium held under the name “Abortion,” a section of psychiatrists (Gautier, Moulin, Gödel and others) met, who unanimously stated that termination of pregnancy is the cause of deep trauma.

Psychological changes that occur after termination of pregnancy

In 1966 The Council of Gynecologists and Obstetricians in England concluded that mental disorders after termination of pregnancy occur, according to various data, in 9-59% of cases. Numerical data are different and indicate imperfection of research. But the statement of practicing gynecologists with extensive practical experience in this area is unequivocal.

Dr. Terrier says that he “never met a single woman who did not have mental disorders after such an operation,” and he even cites cases in which not only mothers, but also fathers showed symptoms of frustration type disorders, which is a very interesting fact for subsequent research.

In Germany, at the University Women's Clinic in Würzburg, Maria Simon, PhD, a clinical psychologist, examined 110 women who had experienced termination of pregnancy in their lives. According to the results obtained, about 60% of the women surveyed felt guilty for what they had done. The feeling of guilt often manifested itself in an altered form, turning into physical suffering. Many women complained of autonomic nervous system disorders that occurred after surgery. Simon believes that in most cases it is the feelings of fear and guilt that are deposited in the secret recesses of the human body, and subsequently manifest themselves as diseases of various organs or neuroses. It looks like the conflict occurs at the somatic level.

Mood changes and depression, which usually occurs after an abortion, were reported by 35-40% of women surveyed. Some had suicidal thoughts that they did not have before the termination of pregnancy. And some women no longer hoped for anything good in the future. They had difficulty forcing themselves to work, although before the abortion they were completely satisfied with their work. Approximately 35% of women often cried for no reason after an abortion, and about 30% felt unreasonably afraid. More than half of the women surveyed felt unhealthy, nervous and unbalanced after the abortion; could not find the lost inner peace. Women were depressed by the very memory of having an abortion, so they tried to find ways that would protect them from feelings of guilt, remorse and depression. For the most part, they imagined abortion as a necessary remedy for the depression that came as a result of an unplanned pregnancy. However, the opposite happened: women only learned what real depression was after an abortion.

Against the objection that the given figures are so high because, perhaps, the women surveyed are mostly believers, M. Simon offers the following arguments: of the 48% of women who consider themselves Christians, 33% do not belong to any denomination and do not go to church. Therefore, the psychological consequences of terminating a pregnancy are not caused by religion, but perhaps by the woman's own conscience.

An interesting statement proposed by M. Simon is that spiritually mature women are more difficult and more consciously experience the abortion conflict. And if they do end the pregnancy, they take it very hard. Spiritually infantile (immature) women are insensitive and do not bear responsibility for their words or actions. They are either hysterical or sad. Such women very rarely treat their unborn child well, so they are more likely than spiritually mature women to terminate their pregnancy. After an abortion, they are more inclined to forget all problems and reduce the conflict to vegetative-neurotic disorders.

The concept of post-abortion syndrome as a psychological condition

The definition of “post-abortion syndrome” was first developed in 1985 by the American psychologist and family therapist Anne S. Speckgard in her dissertation. Professor Philip Ney, a psychiatrist from Victoria (Canada) was also one of the first to describe the mental consequences of abortion in women - post-abortion syndrome. The following researchers scientifically studied and presented to the public post-abortion syndrome (as a trauma after an abortion): S. Stanford, V. Ruhe, A. Specgart, prof. P. Petersen, M. Furch, M. Simon and others.

Post-abortion syndrome (PAS)

is a combination of mental symptoms or illnesses that may occur as a result of post-abortion experiences. The main symptom of the disease is the constant haunting of the experience in the woman’s subconscious.

Sharon Adkins, author of a program for the rehabilitation of women with post-abortion syndrome (the program is called HEART - Healing the Effects of Abortion Related Trauma), uses a slightly different definition:

is a condition characteristic of both men and women after an abortion, which arises due to unresolved psychological and spiritual aspects.

Clinical psychologist Lucy Freed and sociologist Penny Yvonne Salazar use the concept of “post-abortion stress,” a stress response that occurs in women after a pregnancy loss.

The reason for stress in women after termination of pregnancy, just as in the case of PTS, is that they are forced to hide their feelings associated with both pregnancy and abortion. In addition, a woman cannot make up for her loss and find peace of mind.

One of the above definitions of post-abortion syndrome refers to its presence not only in women, but also in men. The nature of its course differs from the nature of this syndrome in women, if only due to the fact that the woman is physiologically connected to the child, she experiences hormonal changes, etc. Among men with post-abortion syndrome, two types can be distinguished: those who wanted a child, but the woman did not agree to give birth to him; those who did not want a child. Therapy for men is more difficult because they do not associate their problems (for example, alcoholism, habit of leaving home, aggressiveness, which can be directed against oneself) with abortion.

The organization Heartbeat International, which develops programs for the treatment of PAS, presents the features of its course - as manifestations of long-term emotional disorder, which include: depression, grief, anxiety, helplessness, despair, regret, low self-esteem, mistrust, cruelty, alcohol/drug addiction /eating/work, guilt, sexual problems, self-judgment, frequent crying, "emptiness", frustration, insomnia, nightmares, bad relationships, flashbacks, anger, fear of rejection, bitterness, unforgiveness, fear of committing oneself bonds, inability to form close relationships.

PAS exposure levels

L. Fried and P. Salazar proposed a model of the impact of abortion on a woman. This model consists of several levels, and the consequences of termination of pregnancy manifest themselves differently in each case. Each personality is woven from several threads intertwined in different ways - psychological, emotional, physical, spiritual, sexual and social (on which our relationships with family, friends and all other people depend).

Psychologically, a woman’s abortion affects her sexual self-awareness, that is, her idea of ​​herself as a woman, because by terminating her pregnancy, she is deprived of the opportunity to play the role assigned to her in society. A woman’s self-esteem also suffers and her self-respect is lost. Many suffer psychologically after an abortion because they no longer perceive themselves as mothers and simply as women. This distortion prevents them from finding satisfaction in love relationships.

An abortion also cannot but affect the emotional sphere, especially the ability to experience feelings. Women often go to extremes. Some completely cut off any emotions. They cease to understand, and frankly, what feelings they experience, or they push true feelings into the depths of consciousness because they are afraid to feel. In public they appear only in a mask, behind which they hide all their emotions. Others go to the opposite extreme: they become emotionally excitable, subject to frequent mood swings. These women perceive everything painfully acutely and take every trifle to heart. These strong emotions break out from time to time, often at the most inopportune times, which makes relationships with others extremely difficult. Both of these extremes lead to the same consequences. Whether our emotions are suppressed or recklessly splashed out, in both cases complications cannot be avoided. All a woman’s mental strength goes into restraining or hiding her feelings. There is almost no strength left to lead a normal life.

It is possible that after an abortion, most women face complications at the physical level. The severity of such disorders can vary - from minor inflammation to infertility. In addition, often, after terminating a pregnancy, women experience physical pain that is not associated with physiological reasons. The connection between emotional stress and illness is already quite well studied: suppressed emotions or stress can cause headaches, back pain, frequent colds, asthma and other so-called psychosomatic phenomena.

People are spiritual beings, and therefore they simply cannot help but have spiritual needs. As a rule, after an abortion, some women try to renounce their spiritual nature altogether, while others try to reconcile their spiritual nature with everyday life.

It is extremely important for every person to develop healthy sexuality. For many women, this task becomes especially difficult after an abortion. They have to deal with opposing views on sexuality, and abortion only adds to this uncertainty. Some women conclude that the best way to solve their sexual problems is to pretend to be non-sexual. They believe that it was because of their sexuality that they became pregnant and were forced to have an abortion. Therefore, it seems to them that forgetting about their sexuality will be much safer than risking getting pregnant again. Often women begin to consciously or unconsciously hide their sexual attractiveness - they dress poorly, gain weight, and do not appear in society. Others suffer because by allowing themselves to become overly involved, they become vulnerable to their sexual partners.

The range of various psychological complications caused by abortion trauma is quite wide. However, some may wonder: are the consequences of abortion really that serious? After all, so many women have abortions and can testify that their health has not worsened after the abortion. The reason for this kind of illusion is that the course of post-abortion syndrome is characterized by three types of reactions.

The first type is an instant reaction. It occurs immediately after the injury or appears within a year. This is due to the fact that already in the first year a woman cannot withstand the tension within herself and “breaks down.” This type of reaction is most favorable for a woman, since the causes of the emotional disorder are obvious, which greatly facilitates diagnosis and therapy. However, the first type of reaction is extremely rare.

The second type of reaction is more common, which is expressed in the fact that painful processes last for many years, they become chronic, but are repressed from consciousness. At times they intensify; most often this happens on the anniversary of an abortion, on the day of the failed birth of a child, etc.

The most common is the third, delayed type of reaction. From five to fifteen years after an abortion there may be no manifestations. Over the years, many women even forget that they once had an abortion in their lives. And suddenly the realization of what they have done literally shocks them, and unexpected symptoms of PAS appear.

The nature of the course of PAS depends on many factors. It is more acute in women during menopause, when she realizes that she will never be able to have children again. Post-abortion syndrome occurs more intensely in religious women, although they are easier to treat.

According to the results of our study in Odessa (2004), it was revealed that the main criteria for the characteristics of a woman’s psychological state after termination of pregnancy are feelings of guilt (72.7%), aggression (63.6%) and fear (81.8). %).
